- Position Summary: Under the supervision of a Registered Nurse, and in collaboration with the Interdisciplinary Team (IDT), provides direct and indirect patient care in accordance with the Nurse Practice Act and organizational policy.
- We visit our patients wherever they may located, whether it be in their homes, long-term care facilities, nursing homes, hospitals, etc. As a result, driving and reliable transportation are requirements of the position.
- Would provide service to patients located in the following Illinois counties: St. Clair, Washington, Monroe, and Randolph.
- Full-Time Non-Exempt Position
- 40 Hours Weekly, Monday-Friday 8:30 A.M. - 5:00 P.M. with an additional rotational on-call schedule that includes some evenings, weekends, and holidays.

- Graduate of State-approved school of nursing.
- Successful completion of Licensed Practice Nurse education.
- One (1) year experience in hospice preferred.

- Current Illinois LPN license.
- CPR certification preferred.
- Excellent clinical assessment, organizational and interpersonal relationship skills.
- A working knowledge of Medicare/Medicaid, hospice regulations, and third-party payor expectations is preferred.
- Reliable means of transportation.
Performs repetitive tasks. Physical tasks including bathing, transferring, lifting, and turning of patients in the home or home-like setting. Prolonged periods of sitting at times. Exerts up to 100 lbs. force occasionally, and/or up to 50 lbs. frequently and/or 20 lbs constantly. Requires physical flexibility of bending at knee and squat-bend. The Staff Licensed Practice Nurse manages stress appropriately, makes decisions under pressure, handles multiple priorities, works alone, and manages anger/fear/hostility/violence of other appropriately. This position requires clarity of vision >20 inches and <20 feet, hears alarms/telephone/tape recorder.
The Staff Licensed Practice Nurse spends approximately 20% of his/her time in an air-conditioned environment with varying exposure to excessive humidity and noise. The remaining 80% of his/her time is spent in an automobile. The position has potential for exposure to malodorous, infectious body fluids from patients, animal pet hair, allergens in the home environment, and some exposure to noxious smells from cleaning agents, exposure to communicable diseases and traffic.

- Collaborates in the development and modification of the Field Staff Nurse/Case Manager comprehensive nursing plan of care for the hospice patient.
- Performs physical assessment and collects data then collaborates with the Field Staff Nurse/Case Manager in the assessment of the health status of a patient.
- Implements aspects of the plan of care, as delegated.
- Participates in health teaching and counseling to promote, attain, and maintain the optimum health level of patients, as delegated.
- Serves as an advocate for the patient/family/caregiver by communicating and collaborating with the IDT, as delegated.
- Participates in the evaluation of patient responses to interventions. Provides direct patient care.
- Participates in on-call after hours care.
- Demonstrates basic LPN clinical skills. Gives a practical exhibition and explanation at the required annual skills day.
- Knowledgeable in patterns of disease progression at the end-of-life and provides ongoing patient/family/caregiver education, as delegated.
- Practices within the guidelines of the Illinois Department of Professional Regulation.
- Provides evidence-based physical, emotional, psychosocial, and spiritual care under the direction of a Registered Nurse.
- Demonstrates knowledge of the principles of end-of-life pain and symptom management.
- Participates in monitoring symptoms and changes in patients and reports these changes to the Field Staff Nurse/Case Manager in charge of patient’s care.
- Participates in on-call after-hour care. In the on-call role, the LPN triages calls and provides care under the direction of a Registered Nurse.
- Participates in IDT meeting by providing information regarding patient’s current health state and observations pertinent to the plan of care.
- Demonstrates the ability to work cooperatively with others in a team environment to ensure deadlines, goals, and objectives are met.
- Demonstrates the ability to embrace change, manage stress, and maintain a positive attitude.
- Builds relationships and collaborates effectively with internal and external customers while cultivating referrals and referral sources.

- Participates in QAPI activities as needed.
