    Overview

    Chestnut Health Systems is seeking a full-time Substance Use Case Manager to work at our Granite City, IL location servicing the Metro East and St. Louis, MO areas.

    Responsibilities

    • Safely transport clients to or from appointments or residential treatment if client is unable to arrange own transportation.
    • Provide active linkage and referral to recommended community resources.
    • Develop and maintain knowledge of addiction, recovery, and co-occurring disorders.
    • Coordinate services with treatment team, referral sources, and other supports to ensure continuum of care.
    • Establish and maintain contact with community agencies, residential facilities, collaterals, and referral sources.
    • Support interventions consistent with the following treatment philosophies: client centered, trauma informed, recovery oriented, and strength based. Be proficient in MET skills.
    • Provide advocacy, both client and system-centered, to ensure that basic client rights are protected.
    • Complete all opening and closing client paperwork for special projects when needed.
    • Maintain thorough documentation of all services provided within the required timeframes.
    • Meet or exceed productivity expectations as determined by supervisor.
    • Demonstrate proficient computer skills to perform job duties.
    • Demonstrate knowledge of and sensitivity to individual’s cultural beliefs and values.
    • Support, model, and teach recovery values including hope, respect, empowerment, health/wellness, and spirituality/connectedness.
    • Function as a team member, attend meetings, client staffings, participate in program development activities, and provide support and backup to other team members as necessary.
    • Participate in professional training and educational activities in order to achieve/maintain professional development and upgrade knowledge/skills.
    • Maintain and protect confidentiality and security of all organizational information gained in the course of performing job responsibilities including, but not limited to, financial and client information.
    • Promote Chestnut’s culture of customer service excellence through the adherence to Chestnut’s behavioral standards for customer service.
    • Perform other duties as assigned.

    Qualifications

    • Position requires a minimum of a high school diploma or equivalent with at least five (5) years' relevant experience;
    • Or a Bachelor’s degree in counseling, social work, psychology or related human services field.
    • Must have a valid driver’s license, private automobile insurance and be insurable.
    • Requires basic computer skills and experience to include MS Word, MS Excel, Outlook, Adobe Acrobat and an electronic medical record (EMR).
    • Satisfactory completion of Chestnut’s background checking process.
