Job Summary:
Provides care and education on disease management to cardiac, pulmonary, and CHF patients in outpatient cardiopulmonary rehabilitation. Provides care to inpatients and outpatients with complex wounds, ulcers, and ostomies. May assist in providing education to patients with diabetes in the hospital setting. May also provide preventative care and skills to the surrounding community through wellness events and disease management programs.

Job Responsibilities:
Education Requirements and Other Requirements:
Education Level: Graduate of an accredited school of nursing; BSN preferred.
Certification/Licensure: Current license with the State of Illinois required. BLS certification required. ACLS certification preferred or within 6 months of hire.
Experience Requirements: Minimum of 2 years Medical/Surgical Nursing required. Experience with cardiac nursing, critical care, diabetes education, and/or wound and ostomy patients preferred.
Working Conditions: Exposure Category I
Task involves exposure to blood, body fluids, or tissues. All procedures or other job-related tasks that involve an inherent potential for mucous membrane or skin contact with blood, body fluids, or tissues, or a potential for spills or splashes of them, are Category I tasks. Use of appropriate measures should be required for every employee engaged in Category I task.
